Incident Reporting Form

The Operations Safety & Security Department at SCC strives to track all incidents that occur on any of our properties across all campuses or centers, whether minor (e.g., theft, vandalism, fender bender in the parking lot) or major (e.g., injuries, crimes). Reporting incidents ensures we have a log for review, required reporting (including Clery Act), and follow-up if necessary.

Anonymous submissions are allowed, but we encourage reporters to provide contact information so we can follow up if needed.
