This video will instruct you on the steps necessary to filling out an online application for Maysville Community and Technical College. This is the first step all perspective students must perform prior to taking classes at Maysville Community and Technical College. We suggest you fill out the online application. However, if you're not familiar with filling out an online form and would rather fill out a paper copy, there is a link to a paper copy. For this video, however, we will be concentrating on how to fill out on online application. Whether you're filling out the online application or the written application, you will still need the same information. Thus, we suggest that you view this video in order to become familiar with the information that you will need to know, whether you are filling out the online application or the written application.
From the Maysville Community and Technical College home page, select the admission link located in the blue ribbon in the middle of your screen by moving your cursor to admissions and using your mouse to select this link. You'll now be taken to the admissions home web page. From the admissions home web page, we can look at the following, such as the admission guidelines for being admitted to Maysville Community and Technical College; how to apply; admissions frequently asked questions or FAQs, as they are more commonly known as. However, in order to apply online to the college, we need to select apply online, which is located in the center of your screen and is written in blue ink, as highlighted here. This is done by placing your cursor over the words apply online written in blue ink and then clicking on it using your mouse. This will take you to the KCTCS application web site. Located on the KCTCS apply online, you will see the words online application written in red ink. Select this link by moving your cursor to the link and selecting it using your mouse. This will now take you to the first question in the online application. Are you a U.S. citizen? Answer yes or no. Our student is a U.S. citizen, so he's going to answer yes by moving his cursor to the box labeled yes and selecting it using his mouse.
You are now taken to the first screen of the application. Notice the number one is highlighted in the upper right-hand corner of your screen. You will need to fill in the blanks after each question. You will move your cursor to the first blank after first name and select it using your mouse. You will now type in your first name. Our student is typing John. Once you've typed your first name, you'll press the tab key, and this will take you on to the next field where you can enter your middle name. Type in your middle name and then press the tab key to move on to the next field. Our student's middle name is Quincy. In the next field, type your last name and press the tab key to move on to the next field. Our student's last name is Student. Any the other names you are known as, such as maiden names if you're married or nickname, type in the name. If there are no other names you're known as, leave this blank and press the tab key to move on to the next field. Our student has no other names he was known by, so he's leaving this blank. The next field asks you to type in your social security number. John is entering his social security number and then pressing the tab key to move to the next field. You are now asked to enter the date of your birth. The format you will need to use is two digits for month, two digits for day, and four digits for year. John has entered 01 slash 31 slash 1990 as his date of birth. He will then press the tab key to move on to the next field. Next, the application asks you to list your gender. This is selected by selecting a choice from the drop-down menu. Note that gender has a plus sign in front of it. And if you read the note at the beginning of the page, those items with a plus sign before them are optional questions. You do not have to answer them if you do not choose to. John has decided he will answer this question, so he picks male from the drop-down menu and then presses the tab key to move on to the next question. The next question is asking you to enter your ethnicity. Again, this is an optional question that you can select from the drop-down menu to see the available options. John has selected native Hawaiian Pacific Islander because his family is originally from Hawaii, although he's lived most of his life in Kentucky. He will then press the tab key to move on to the next question. The next question is asking him if he's been in the military or currently in the military or have not served in the military at all. Again, use the drop-down menu to select the answer. In John's case, he's never been in the military. He then presses the tab key to move on to the next question. In the next question, John will answer no because he wasn't in the military. He will select no to answer the question, will you be receiving veteran's benefits, yes or no. If you will be receiving veteran's benefits, you would want to answer this question yes and then press the tab key to move on to the next field. The next question asks John if he's applying for financial aid. He answers this question with yes by selecting the answer and pressing the tab key to move on to the next field. We suggest you watch the video on financial aid. This video can be accessed from the student services web site. The next question asks you to enter your current email address. This is very important in case the college needs to contact you for processing your application. John enters his email address as John dot student at hotmail.com and then presses the tab key to move on to the next field. The next field asks you to select your phone type. This is the main phone where you can be reached during the day. Select from the drop-down menu. John has chosen to give his cell phone number, because that's the easiest number he can be contacted at during the day and then press the tab key. He enters his cell phone number. He types 606-584-5555 and then presses the tab key, which takes him into typing in his complete address. He enters 1234 Maple Leaf Drive. This a house; but if it was an apartment, he would also enter the apartment number. He then presses the tab key, moving on to the ZIP code field. John then types in his ZIP code. In this case, it is 41056. Notice how he's automatically taken to a screen that states, Based on the ZIP code that you've entered, is this the correct city, state, and county? In this case, John entered 41056. It asked John if the city of Maysville, state of Kentucky, and the county of Mason is correct, yes or no? This is the correct information. So he selects yes, which returns him back to page 1 of the application and automatically enters the information into the necessary field. Page 1 is now complete. We suggest you review it and make any changes and then press continue to go on to page 2 of the application.
On page 2, the first question asks you to select the KCTCS campus you will be attending. Use drop-down menu to select the campus. John is attending the Maysville campus. So he'll scroll down to find Maysville and select it by placing his cursor over it and using his mouse to select it and then presses the tab key to move on to the next question. The next question asks John to enter a starting term. He plans on entering in spring of 2010, so he used the drop-down menu to select the semester he would like to start attending Maysville Community and Technical College. In this case, he chooses spring of 2010, which runs from January through May of 2010 and presses the tab key to move on to the next field. This field asks John to enter a description that best describes you as a new student applying to Maysville Community and Technical College. By selecting from a drop-down menu, in this case John selected he is a first-time college student but he earned some college credits while he was in high school. He then presses the tab key. The next field asks you to enter your area of study. John would like to study engineering, so he selects engineering technologies technician by using the look-up feature that's located next to the question. Press the tab key to move on to the next question. The next question asks you to enter your academic plan. John again uses the look-up feature to choose an academic plan. In our case, John has chosen engineering technology and presses the tab key to move to the next field. The next question asks John to enter your academic subplan. He then uses the look-up feature again and chooses MIT engineering technology industrial plan, because he wants to get his degree in industrial technology engineering. He then presses the tab key to move to the next field. The next question asks John about his residency status for the past 12 months. It asked, have you lived in Kentucky for the past 12 months, yes or no? So he chooses yes from the drop-down menu and then presses the tab key to move on to the next question. The next question asks John, Have you received income from outside Kentucky in the last 12 months, yes or no? He answers this question with a no and presses the tab key to move on to the next question. The next question asks John, Does your parent or legal guardian live in Kentucky, yes or no? Both his parents still live in Kentucky. And he answers yes and then presses the tab key to move on to the next question. The next question asks John, Are you a spouse or dependent of an active duty military member, yes or no? He answers no, because he is not married; and no one in his recent family has served in the military. He now presses the tab key to move on to the next question. He will then press continue to go on to page 3.
By placing his cursor over the box entitled continue and selecting it using his mouse, John is now taken to page 3 of the application. The first question on page 3 asked if you have your high school diploma, answer yes or no. In John's case, he has his high school diploma. So he selects yes and then presses the tab key to move to the next question. The next question wants to know your high school graduation date. You need to enter it in the follow format: two digits for month, two digits for day, and four digits for year. In John's case, he enters 05 25 2008. He then presses the tab key. Next, you are asked what country's your high school in. In John's case, his high school was in the United States. So he selects U.S.A. from the drop-down menu and presses the tab key to move on to the next question. He is then asked what state was his high school located in. He uses the look-up menu, which takes him to another page; and he selects Kentucky as his state his high school was located in and then presses the tab key to move on to the next question. He is then asked what city was your high school located in. John's high school was located in Maysville. He uses the look-up feature to select Maysville and presses the tab key to move on to the next question. The next question after that asks John to indicate the code for his high school. John does not know the code for his high school, so he moves his cursor to the look-up feature and is given three choices to choose from. He selects Mason County High School and then presses the tab key to move to the next question. The final question on page 3 asks if you attended a Kentucky Technology Center. John clicks on no because he did not. He reviews the answers to the question asked on page 3. His answers are correct. To move on to the next page, he moves his cursor down to the continue button and selects continue using his mouse. He is now taken to page 4 of the application. The first question John is asked is whether or not he previously attended college. If so, where and what dates did you attend. In John's case, he did previously attend another college. Remember, he indicated he was a first-time student but had received some college credits while attending high school. He will also need to get an official copy of his transcripts from the other college he took the courses from sent to Maysville Community and Technical College. He presses the tab key to continue. Next it asks him what country was the college located in. He can either type in U.S.A. or use the look-up feature to find the listing for U.S.A. and then presses the tab key. The next question asks in what state was the college located. He selects Kentucky from the look-up feature. He then needs to select the city. John realizes that the college he took courses was not located in Kentucky. It was actually located in Portsmouth, Ohio. So he needs to go back in and change the state in which the college was located. So he cancels out of the city screen and returns back to the state entry by moving his cursor back to the state entry box and selecting it using his mouse. He is able to change the state to Ohio and presses the tab key to move on to the next question. Again, he's at the question asking him to indicate which city the college he attended was located in. He then chooses Portsmouth as the name of the town. Notice when you go to various look-up screens, you can either use the search mechanism located at the top to narrow down your search. In this case, John types in the letter P and presses enter on his keyboard. He is now taken to listing for cities beginning with P located in Ohio. He then scrolls down until he finds Portsmouth and then selects it using his mouse.
He is now taken back to page 4 of the application. He then presses the tab key to move on to the next question. The next question asks him to enter his school code. John does not know Shawnee State's school code. So he moves his cursor to the look-up feature and selects it using his mouse. Based on his previous answer of Ohio and Portsmouth, all the colleges in Portsmouth are now displayed. There is only one college listed there, and it's Shawnee State. So he selects it by placing his cursor over the university's name and selecting it using his mouse. The school code and the name of the school are automatically entered into the school code and the name of the school field. He then presses the tab key to move on to the next field. The next question on this page asks John to enter the start date and the end dates regarding his attendance at Shawnee State University. John enters that the starting date that he started taking classes at Shawnee State in the format of two digit for month, two digit for day, and four digit for year. For example, he entered in 01 slash 31 slash 2008 to 12 01 2009. He then reviews the information on page 4 to make sure it's correct. He then moves the cursor to the link that says continue and selects it using his mouse. He is now taken to page 5. This is the final page that shows all the information that John has entered in the application. Make sure you take the time to review the information and confirm all the information is correct before submitting the application. When you are sure the information presented in the application is correct, move your cursor to the continue button at the bottom of the screen and select continue by using your mouse. By pressing continue, you've now completed an online application for Maysville Community and Technical College.
