First, let's take a look at the financial aid forms. You can select this by placing your cursor over the financial aid forms heading from the menu located on the left hand side of your screen and select it using your mouse. There are several forms to fill out. The first form that needs to be filled out is the FAFSA form and this form can be completed by going to www.fafsa.ed.gov. Let's take a quick look at the financial aid forms offered to Maysville Community & Technical College. The first form listed is the scholarship application. This is an excellent form for you to fill out for scholarship funds. These are typically funds that you do not have to pay back. This application can be completely filled out online and submitted to our financial aid department. Use the back arrow at the top of your screen to return to the previous screen by placing your cursor on the back arrow and select it using your mouse.

The next form is the financial aid release form. Select the form by moving your cursor over the title heading, FAR Release, and select it by using your mouse. The financial aid release form comes up; this form is a release for allowing your books to be paid for by your financial aid disbursements. The Title IV release allows any of your financial aid disbursements to pay for any additional charges such as library fees, parking fees, lab fees, et cetera. FERPA allows the college to release information about you to your either parents or your spouse or both. This form needs to be printed and filled out and returned to the financial aid department located in the student service office. Press the back arrow at the top of your screen to go back to the previous screen. This takes us back to the financial aid forms home page.

The next few forms are to be utilized when using your FAFSA form, the verification worksheet, the worksheet A, B, C for verification. The Stafford Loan Application; the recalculation sheet is done if your financial circumstances change. The FAFSA form is based upon the previous year's income. A good example for needing your financial status recalculated is for the year you've lived on your own and supported yourself. But the FAFSA form requires you to report your parent's income even though you have been supporting yourself and do not receive financial support from them. You may be able to have your financial aid recalculated based on your income for the previous year. The satisfactory academic progress report--this is a report that students would use if they have lost their financial aid for some reason. You have the right to appeal and try to regain your financial aid. This form is filled out by the student and returned to the campus financial aid office for processing. Press the back arrow to the financial aid report form home webpage. The next report is the financial aid degree audit. This is used by students who have taken longer to complete their degree than anticipated and are in danger of losing their financial aid. To return back to the financial aid form, place your cursor on the back arrow and select it using your mouse. The next report we'll take a look at is the KCTCS Temporary or student employment application. This is a form used by a student who would like to perform work study hours while attending a college. Please fill out this form and fill it out and return the filled out form to your campus financial aid office. To return back to the financial aid form main menu, press the back arrow located at the top of your screen by placing your cursor over it and selecting it with your mouse. The final form we will look at is the KCTCS Consortium Agreement. This is an agreement that would be filled out if the student is attending more than one college at the same time. This creates an agreement for accepting the credits from the other college.
